Patient Registration
All new patients are required to complete a registration form. To save time, you may wish to print & complete the registration form prior to your appointment. You can bring the completed form with you on the day or email to: office@paso.com.au
It is important to have a current referral. If you do not have a current referral you will be required to pay in full on the day of the consultation. Our reception staff can assist with any queries regarding your current and new referral requirements.
GP Referrals are valid for 12 months. A new referral is required if your GP has changed or moved locations. Specialist Referrals are valid for 3 months.
